Your 1095-C is available to you upon request IF you are either:
• Full-time according to IRS guidelines, or
• Enrolled in the medical plan during the 2024 plan year.
Requests can be made by the following:
• visiting your local IMKO office
• calling us at 1-816-233-4040
• sending an email to healthcare@imko.com. (If requesting by email, please
include your full legal name, the last 4 of your SSN, and your DOB.)
We will send the requested form within 30 days. Additionally, the form may be
available electronically for a faster response time.
